A form from the County Clerk’s Office must be completed. Or you can download, print and complete (all but signatures that have to be signed in front of a Notary Public) the form:

If there is more than one owner, all owners must sign the business registration form, in the presence of a Notary Public.
If the business is owned by a Corporation, a copy of the corporate papers is required (first and last pages only, with signatures of corporate officers.
If registering the name of a franchise, include a copy of the Franchise Agreement (first and last pages only).
