Birth records are not public records and can only be obtained by the individual or the parents of the individual either in person or through the mail.

 

In Person:
Submit driver’s license or three pieces of I.D. including at least one picture I.D.

 

By Mail:
Download and print off the attached Birth Certificate Application including father’s name, mother’s maiden name, and a copy of driver’s license to:

Maple Ridge Township has begun the process of updating its 5-Year Recreation Plan. The plan should reflect the community’s needs and priorities so input of the residents is necessary.  Survey forms can be obtained and dropped off at the Maple Ridge Town Hall and the Rock Credit Union.  The Township also has the survey posted online.
